Advance Directives

Advanced Directive is a document executed in advanced of a patient incapacitation, by which a competent person states his/her medical treatment choices and/or another person to make decisions. During the registration process, the Patient Access Clerk will assist the patient or family member in completion of the “Conditions of Admission” form, asking all individuals 18 years of age or older receiving inpatient, observation, outpatient surgical care, outpatient endoscopic procedures and invasive radiological procedures if they have executed an Advance Directive. The Patient Access Clerk will also complete the Advance Directive tab in the Patient Management module of Cerner, which will alert Social Services via the Advance Directive Listing report of patients in need of their assistance.
